New Student Immunization Rules
The Tennessee Department of Health (TDOH) has issued new immunization rules and a new Official Immunization Certificate (Form # PH-4103) that is required for school entry into certain grades.
This applies to pre-school and pre-K where 4 new immunizations are required. Kindergarten will require additional doses of existing immunizations. Students currently enrolled in Robertson County Schools who will be entering 7th Grade will be required to submit the new Official Immunization Certificate documenting the 2 newly required immunizations. All new students to a Tennessee School will be required to provide proof of immunizations on the new Official Immunization Certificate at time of enrollment. The newly required immunizations will not be available at school. The immunizations and new Official Immunization Certificate will only be available from a private healthcare provider or the local health department beginning April 1, 2010. Required physicals for school entry must also be documented on the new certificate. Old immunization certificates will no longer be accepted and the new Official Immunization Certificate showing proof of immunizations must be provided at time of registration for those grades as stated above.
